New Patients
New patients can expect to fill out electronic forms including a Patient History and a Notice of Privacy Practices. This information helps ensure that we provide you with the best possible care. You can save time at your first visit by completing the necessary forms in advance of your appointment. To get started, click below.
Initial Exam
With the exception of emergency visits, you can expect your initial exam to include:

a complete medical history, please bring a list of any medications

a clinical head and neck exam, including soft tissues and teeth-oral cancer screening

recording a baseline of all existing restorations and oral conditions

xrays to detect decay and evaluate supporting bone
-
periodontal screening (“probing”) - measuring the tissues that surround the neck and root of the tooth
-
time to ask any questions/concerns you have about your oral health
-
time to get to know your doctor
After collecting all of this information, we can decide on the right treatment plan for you. This plan includes what treatment is necessary, why it is necessary, how much it will cost, and an estimate of insurance coverage (when applicable). In cases where there are multiple treatment options, all options will be discussed.
Payment Options
At Marshalltown Family Dentistry, we understand that dental care can be expensive, but we want you to have every opportunity for the best dental care. We offer the following options to make dentistry more affordable:

MasterCard, Visa, Discover, American Express

Insurance (as a courtesy, we will file your claim for you)

CareCredit (6 month payment plan)
We do collect payment for treatment the day of service. If you have dental insurance we will estimate your co-payment based on the insurance company information supplied. If there is a remaining balance after billing to insurance, we will bill you for the remainder.
